Transcript Request

Students must request a transcript in writing using the form provided on the link below or the forms available in the counseling center.  Students may also request a draft copy to be sent to them as well.  Each request is $2.00.  Transcripts requests will be processed within a five day time period.  If the student knows the contact for the university, college or scholarship that he/she is applying, please include this information as well on the request form.
